Notice of Privacy Practices
The HIPAA Notice of Privacy Practices describes how the practice may use and disclose your protected health information, and your rights to access, amend, and request restrictions on it.
The notice is issued by South Texas Spine and Surgical Hospital. A copy is given to you at your first visit, and the office will send one on request: call 210-293-9380.
